Appointments

How can I make an appointment?

PROV 205 offers you the convenience of requesting an appointment via the 'Appointment request' link on our home page.  You may also schedule an appointment by calling the office at 201-455-2052. 

What do I need to bring to the first appointment?

All new clients need to provide photo identification and health insurance cards.

Do you accept my health insurance plan?

PROV 205 is an out-of-network provider and accepts most private insurances with out-of-network benefits. *The office does not accept Medicare or Medicaid.

As a courtesy, we will attempt to verify your insurance eligibility before receiving services. However, it is your responsibility to verify your coverage and benefits, including but not limited to policy provisions, deductibles, limitations and authorization requirements. The office can file insurance claims for the client, however co-insurance, deductibles, or non-covered services must be paid at the time of service. Lastly, clients may receive financial statements for any balances due.

If the office cannot process the claim to your private insurance, full payment will be required. A superbill will be provided for you to submit to your private insurance for reimbursement.

Do you accept self-paying clients?

Yes.  We offer services to children, adolescents, adults, couples, families and groups.  Rates will vary depending on the service type and number of people seen at one time.

What is the cost for the visit?

We provide multiple services.  Please contact our office to obtain the specific rate for the specific service needed.    

Do you have a cancellation policy?

Yes, clients must contact the office at least 24-hour in advance to cancel their appointment and avoid the cancellation / no-show fee. The cancellation / no-show fee will be 50% of the appointment fee.

How long is each therapy session?

Sessions are 50 minutes long.  However, the appointment time may vary depending on the type of visit: individual, couple, group, psychological or immigration evaluation.

Can I bring someone with me?

Yes, clients are encouraged to visit the office with someone who can provide them support. However, during the time of global pandemic we are asking clients to attend alone.

How many sessions will I need with the psychologist?

It is difficult to determine the length of treatment needed for any particular client. The APA recommends clients begin treatment with a series of at least 6 - 8 sessions. Nonetheless, in the initial session, the clinician will recommended the type and length of treatment required.

We accept cash, personal checks, Visa, Master Card and Discover.
